Community Health Worker – CRC
Job Summary : The Community Health Worker (CHW – CRC) with Choptank Community Health System (CCHS) is responsible for acting as a bridge between healthcare providers, community services, and patients to increase colorectal cancer screening rates among Choptank Community Health patients. Using culturally responsive, patient‑centered strategies, the CHW supports patients in understanding the importance of colorectal cancer screening and assists them in overcoming barriers to screening completion through education, reminders, navigation, and care coordination.
This position focuses on targeted outreach, patient reminders, and navigation activities to ensure appropriate implementation, utilization, and data capture related to colorectal cancer screening at the clinic level. CHW-CRC works closely with clinic teams, population health staff, and providers to support screening workflows and improve preventive cancer care outcomes.
This is a non-exempt, full-time position in pay grade 3 with the pay range of $19.14 - $24.88. The Community Health Worker - CRC - reports directly to the Director of Quality & Population Health. This is a grant-funded position.
Required Skills/Abilities:
- Must be able to work independently and have excellent problem-solving skills to provide patients with optimal assistance.
- Excellent communication and interpersonal skills with the ability to work well under stressful conditions to effectively interact with patients, families, and various programs when obtaining necessary information.
- Must have working knowledge of medical terminology and word usage to ensure accurate referrals and concise, detail-oriented performance to ensure optimum referral tracking and follow up.
- Basic computer skills, including Word, Excel, and Outlook
- Must maintain a high level of confidentiality.
- Must have own transportation and ability to travel to CCHS sites on a weekly basis.
Education and Experience:
- High school graduate or equivalent.
- CHW or Social Work background preferred.
- CHW certification will be required but can be obtained after starting employment.

Duties/Responsibilities:
- Provides targeted outreach to patients overdue or at risk for colorectal cancer screening
- Educates patients on colorectal cancer prevention and screening options using culturally and linguistically appropriate approaches
- Conducts patient reminders (phone, text, mail, or in‑person) to support screening completion
- Assists patients with scheduling screening appointments and addressing barriers such as transportation, health literacy, fear, or access issues
- Provides patient navigation support across the screening continuum, including follow‑up for abnormal results as directed by clinical staff
- Collaborates with clinic teams and Population Health staff to ensure proper screening workflow implementation
- Supports accurate documentation, tracking, and data capture related to cancer screening activities
- Participates in team meetings, quality improvement efforts, and grant reporting support as needed
- Assists patients navigating the health system by connecting patients with needed services, filling out paperwork, and providing non-directive emotional support.
- Improves quality of care by aiding communication between provider and patient to clarify cultural practices and to promote wellness by providing culturally appropriate health information to patients and providers.
- Regular, reliable attendance is a requirement of this job.
