Chicago House
Chicago House is seeking a Housing & Health Coordinator - Older Adults Specialist. The role focuses on integrated housing navigation and care management to improve housing and health outcomes for adults 50+. You will work with Chicago House teams and external medical providers to support aging in place and stable housing.
