Champion Multi-Disciplinary Coordination: Act as the central point of contact for the entire care team, synchronizing specialists and support staff to provide a seamless, holistic patient experience. Translate Clinical Orders into Action: Execute physician and nursing directives with precision, ensuring that treatments are delivered safely and effectively. Facilitate Safe Transitions: Evaluate and coordinate discharge needs early in the stay, working closely with the healthcare team to ensure patients have everything they need for a successful return home. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
