Job Summary We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team. As a valued member of our healthcare team, you will play a crucial role in delivering high-quality patient care to our residents. In this role, you will work collaboratively with other healthcare professionals to provide exceptional care and attention to our patients' needs. Responsibilities Provide direct patient care to residents in accordance with established care plans and protocols Administer medications, perform wound care, and provide basic wound care to residents as needed Monitor and report any changes in patients' conditions to the nursing staff and healthcare providers Collaborate with other healthcare professionals to develop and implement individualized care plans for residents Maintain accurate and up-to-date records of patient care, including progress notes and medication administration records Participate in quality improvement initiatives to ensure high-quality patient care About This Opportunity Working Schedule: Evening shift, Monday to Friday, 3:00 pm to 11:00 pm Work Environment: Our state-of-the-art rehabilitation center provides a warm and welcoming environment for our residents and staff.
