Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years in Acute Case Management in California License: Valid Registered Nurse License in California Certifications: Applicable certifications preferred Must-Have: - Experience in Acute Case Management (Outpatient, Home Health, or Health Plan experience is not sufficient) - Strong verbal and written communication skills - Excellent time management skills - Ability to work independently and exercise sound judgment - Commitment to service excellence - Ability to promote teamwork Description: The Case Manager RN will be responsible for managing patient care within a managed care environment. Candidates should have a broad knowledge base of healthcare delivery and case management. Comprehensive knowledge of Utilization Review, levels of care, and observation status is required. Awareness of healthcare reimbursement systems is preferred. Candidates should have a working knowledge of laws, regulations, and professional standards affecting case management practice. A general understanding of coding and DRG assignment processes is also preferred. Candidates must effectively communicate and promote cooperation among patients, families, physicians, nurses, and other partners. The ability to interact effectively with internal and external constituents using collaboration and customer service skills is essential. Demonstrated commitment to quality patient care and a positive patient experience is crucial. Candidates should possess strong negotiation skills and be able to develop organized work processes in a high-volume environment with rapidly changing priorities. Intermediate computer and technology skills are required.
