Case Manager (MA/MSW)

Responsibilities Valle Vista Health System is a 132‑bed acute care psychiatric hospital located in Greenwood, IN. The facility features individual units for adolescents and adults and offers inpatient acute care as well as a variety of outpatient services at multiple locations. On average, over 10,000 patients receive care from our compassionate health‑care team each year. Valle Vista is proud to be a teaching facility that supports local nursing students and other behavioral‑health professionals. The Case Manager (CM) provides services to our acute adults and/or adolescent patients with psychiatric and chemical‑dependency needs. As part of a treatment team led by a physician, the CM assesses patient needs, develops individualized plans, and provides group and individualized care. The CM’s work with patients is focused on returning them to a level of functioning that allows community living with individual support systems in place. Job Duties Individual and group therapy Psychosocial assessments and treatment planning Child and family team meeting facilitation Qualifications Education: Master’s degree from an accredited college or university in Social Work or a clinically related mental‑health field. License: State‑licensed in social work or a clinically related mental‑health field; LCSW preferred. Experience: Previous experience in a psychiatric health‑care facility with direct experience in family, individual and group therapy, case management, and crisis intervention preferred. Trauma‑Focused CBT and DBT experience preferred. Additional Requirements: Must be at least 21 years of age.
