The Advanced Practice Provider will provide comprehensive primary health care service to patients in the prevention, diagnosis and treatment of disease. Acts as primary health care provider with the scope of his/her primary care specialty. Performs physical examinations on new and returned clients. Refers patients to specialists as necessary. Documents all patient care activities. Maintain a productivity level necessary to reach mandated annual level of encounters. Main Duties & Responsibilities Performs history and physical on new/return patients throughout the lifespan. Orders appropriate diagnostic/screening test and reviews test to establish diagnosis; initiates or continues treatment based on findings. Manages acute and chronic illness according to established protocols and/or in collaboration with physicians. Provides follow-up health care providers or community agencies as necessary.
