This role involves patient education primarily via phone, with occasional in-person sessions for glucose meters and Ozempic shot instructions. You'll also handle prior authorizations, refill requests, and lab orders, ensuring seamless patient care.

• Provide patient education via phone and in-person as needed
• Manage prior authorizations and call in prescription refills
• Coordinate lab orders and ensure accurate documentation in EPIC EMR
• Collaborate with a dedicated team to enhance patient outcomes

Requirements for this role include:

• EPIC EMR experience is REQUIRED
• Need 1 year experience in setting
• Strong communication and organizational skills
