Care Coordinator

Job TitleEssential ResponsibilitiesCoordinate daily, weekly, and ongoing schedules for Personal Care Aides (PCAs), Home Health Aides (HHAs), and other field staff.Match caregivers with client cases based on availability, qualifications, location, client needs, and agency requirements.Fill open shifts and address call-offs, cancellations, late arrivals, and other staffing changes.Communicate schedule changes to caregivers, clients, families, and appropriate agency departments.Maintain accurate scheduling information within the agency's scheduling or electronic health record system.Maintain awareness of caregiver availability and help maximize staffing coverage.Work collaboratively with Recruitment, Human Resources, Clinical, Coordination, and Operations departments.Participate in after-hours or on-call scheduling responsibilities when required.Provide professional and courteous customer service to clients, caregivers, families, and internal staff.Protect confidential client and employee information in accordance with agency policies and applicable regulations.Perform other duties as assigned.QualificationsHigh school diploma or equivalent required.Previous experience in home care, healthcare scheduling, staffing, customer service, or a related field preferred.Experience working with PCAs, HHAs, caregivers, or home health staff preferred.Strong organizational and time-management skills.Excellent verbal and written communication skills.Ability to multitask and prioritize in a fast-paced environment.Strong problem-solving and conflict-resolution skills.Ability to work independently while also functioning effectively as part of a team.Proficiency with Microsoft Office and electronic scheduling or healthcare software.Ability to maintain confidentiality and handle sensitive information professionally.Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
