Responsibilities

• Provide in-home nursing care and clinical assessment to patients.

• Develop and update patient care plans with measurable goals.

• Monitor and document patient progress; adjust care as needed.

• Ensure compliance with safety, privacy, and regulatory standards.

• Maintain accurate, timely documentation in electronic health records.

• Coordinate with physicians and interdisciplinary team for care decisions.

• Educate patients and families on treatment, medications, and safety.

• Utilize data and pipelines to identify high-potential patients and optimize visits.

• Meet or exceed quality and productivity targets through efficient scheduling.

• Demonstrate professional development; pursue ongoing learning opportunities.

• Engage in community outreach to facilitate referrals and partnerships.

Experience/Qualifications

• Current RN license and BLS certification in home health settings.

• Proven proficiency with electronic health records and home health documentation.

• Ability to develop and update individualized care plans with measurable goals.

• Strong clinical assessment skills with regular patient progress documentation.

• Effective in coordinating interdisciplinary team and physician communications.

• Experience with risk management, safety, privacy, and regulatory compliance.

• Demonstrated achievement of quality and productivity targets under pressure.

• Commitment to ongoing professional development and learning opportunities.

• Data-driven approach to identify high-potential patients and optimize visits.

• Strong patient and family education skills on medications and safety.
