These personalized services include medication reminders, meal preparation, bathing or showering care assistance, light housekeeping and laundry, help with transfers or mobility, companionship and respite care, and help with errands and transportation.

Job Description Summary

We are seeking a compassionate and experienced Client Care Coordinator to support our caregiving team and ensure exceptional care for our clients.

Client Care Coordinator combines the essential duties of their caregiver role and leadership.

The Client Care Coordinator will play a critical role in opening new client cases, managing complex or escalated care situations, and training and mentoring new caregivers. This position combines leadership, direct client interaction, and administrative coordination to uphold the highest standards of in-home care.

In addition to the Client Care Coordinator duties; you will provide non-medical service and assistance to a client in their home and communities, who, because of advanced age or physical or mental disability, cannot perform these services. These services enable the client to remain in his or her residence and include, but are not limited to, assistance with the following: bathing, dressing, feeding, exercising, personal hygiene and grooming, transferring, ambulating, positioning, toileting and incontinence care, assisting with medication that the client self-administers, housekeeping, meal planning and preparation, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship.

Client Care Coordinators are responsible for ensuring that service is delivered in a caring and respectful manner, in accordance with relevant Agency policies, industry standards, and state regulations.

Minimum Eligibility Requirements

Qualifications include:

18 years of age or older;

Clearance Verified on the Home Care Aide Registry;

Current Tuberculosis (TB) certificate;

Valid I-9 Employment Eligibility Verification documents;

Completed "Criminal Record Statement" (LIC 508);

Complete "Personnel Record" (LIC 501)

Complete "Statement Acknowledging Requirement to Report Suspected Abuse of Dependent Adults and Elders" (SOC 341A)

Minimum of five (5) hours entry-level training prior to presence with client(s).

Essential Job Functions/Responsibilities

Assist with the activities of daily living and personal care including:

  • bathing - shaving - ambulation
  • mouth care - dressing - exercising
  • hair care - feeding - toileting/incontinence care
  • nail care - positioning - medication reminding
  • skin care - transferring - meal planning & preparation

Work directly with other departments to maintain high level of service for clients by performing various tasks
