Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to their unique needs
• Implement physical therapy interventions focused on enhancing mobility, strength, balance, and overall function
• Engage in collaborative efforts with nurses, physicians, case managers, and other rehabilitation experts to ensure optimal care and outcomes
• Educate patients and their families on therapeutic objectives, discharge plans, and home exercise programs
• Maintain comprehensive documentation of evaluations, treatments, and patient progress according to facility standards and regulations
• Be an active participant in multidisciplinary team rounds and conferences to facilitate coordinated care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• Preferred CPR certification
• Additional certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Previous experience is highly desirable
• Ability to work with patients facing complex medical challenges and mobility limitations
• Excellent communication and collaborative skills are essential
