LVN/LPN Home Health

Job Summary:

Provide skilled nursing care in a home health setting within the Greater Manchester area, ensuring quality patient care and compliance with agency protocols. Deliver services through scheduled home visits, documenting and managing patient needs effectively.

Responsibilities:

• 
  Administer nursing care to adult patients in their homes according to physician orders and agency policies.
  
  
• 
  Complete all documentation within 24 hours of patient visits, ensuring accuracy and completeness.
  
  
• 
  Travel up to 75 miles daily to visit patients, managing a caseload of 5 to 8 patients per day.
  
  
• 
  Collaborate with team members during periodic office meetings and coordinate patient care plans.
  
  
• 
  Maintain current CPR and BLS certifications, and ensure license validity at all times.
  
  
• 
  Follow productivity expectations of approximately 7 patients daily, totaling 30 points weekly.
  
  

Qualifications:

• 
  Possess a valid LPN license in the state of New Hampshire.
  
  
• 
  Have at least one year of recent adult home health experience, or hospice experience for relevant roles.
  
  
• 
  Proficient in use of the Homecare Homebase (HCHB) EMR system within the past six months.
  
  
• 
  Hold current CPR and BLS certifications.
  
  
• 
  Demonstrate reliable transportation, a valid drivers license, and auto insurance.
  
  
• 
  Willing to travel up to 75 miles from the assigned care center.
