This is a highly independent and responsible Advanced Practice Registered Nurse position with the HIV/Specialty Care Clinic. This position is part of a multidisciplinary team that ensures proper care and treatment to clients who are seen for the following services: HIV/AIDS and Community Outreach. Your specific responsibilities include providing services for people living with HIV/AIDS as part of a team delivering enhanced care coordination in Lake County, with the goal of improving engagement in care, increasing retention, and supporting viral load suppression. The practitioner must be skilled in primary care, disease processes, and evidence-based practice, and must apply advanced nursing principles in the care and treatment of clients, including prescribing and administering medications and ensuring accurate documentation.
The practitioner must be able to conduct thorough assessments, diagnose and manage both acute and chronic conditions, and deliver evidence-based treatment with a focus on prevention, wellness, and patient education. Because this role serves individuals with sensitive communicable disease issues, the incumbent must be comfortable interviewing clients about HIV/AIDS and high-risk activities. This position includes providing holistic care and must be able to diagnose and treat various secondary and opportunistic infections like sexually transmitted infections, hepatitis, and tuberculosis. Services can include initial and annual physicals, performing well-woman exams such as pap smears, pelvic exams, and breast exams when appropriate.
HIV-specific responsibilities include reviewing lab results and providing follow-up on abnormal findings—especially viral loads and CD4 counts—while assessing medication adherence, identifying barriers such as missed appointments or issues with housing and transportation, and implementing needed clinical recommendations. The practitioner provides care coordination, especially for newly diagnosed clients, and serves as a liaison with case management, specialty care providers, and State Telehealth Providers. The incumbent is also responsible for reviewing and authorizing prescription requests based on client needs. Additional duties include evaluating immunization status, administering vaccines per protocol, and documenting all immunizations and clinical services in Florida Shots, the Health Management System (HMS), and the Provide Enterprise System. Medication refill requests must be entered into HMS–Emdeon, and all client encounters—whether face-to-face, via telehealth, or by phone—must be entered into the required systems for tracking and reporting.
The position requires strict compliance with confidentiality protocols and database security standards of the Florida Department of Health HIV/AIDS Section and related systems such as Medicaid/FLMMIS, HMS, Availity, Change Revenue/Optum, STARS, and Florida Shots. The incumbent must submit reports, maintain records, and complete paperwork—including timesheets, mileage, and EARS—accurately, legibly, and on time. Effective communication is essential, including answering calls promptly, speaking clearly and professionally, and assisting with clerical functions when needed. The role includes participation in HIV treatment adherence programs, HIV/AIDS education and prevention trainings, and care management workshops or webinars. The incumbent provides training and information to DOH Lake staff, community-based organizations, and case managers, and participates in outreach efforts such as health fairs, community events, and provider collaborations to promote available services. Staying current with HRSA policies and procedures is also required. Quality improvement responsibilities include assisting with chart reviews, engaging in QA/QI activities to support DOH-Lake's mission, and participating in the Quality Advisory Committee. The practitioner also takes part in case discussions, case conferences, ADAP and DOH-Lake meetings, and collaborations with community partners, while maintaining relationships with Key Points of Entry into the healthcare system. Meeting minutes must be transcribed and distributed within two days. The position supports the Program Manager by assisting with pharmacy coordination, pilot studies, surveys, focus groups, and reports, and must report any client concerns immediately to the Manager or designee. The incumbent may be required to work before, during, or after emergencies, including in special needs shelters or Red Cross shelters, and must maintain confidentiality and the security of all records as required by departmental regulations and laws.
Required knowledge, skills, and abilities include bilingual preferred (English/Spanish preferred), knowledge of HIV/AIDS and STD, terminology, principles and analytical techniques of interviewing, data collection and analysis principles and procedures. Customer service skills. Ability to prepare reports, understand and apply applicable rules, regulations, policies and procedures relating to Ryan White, ADAP State and Local Specialty Care HIV/AIDS Programs, understand and follow legislative policy direction as established in Federal and Florida Statutes; deal with the public in a tactful and courteous manner; work independently, plan, organize and coordinate work assignments, communicate effectively, establish and maintain effective working relationships with others; utilize problem solving techniques and use personal computers.
Qualifications include currently holding an active Florida Advanced Practice Registered Nurse (APRN) license. Verifiable experience as nurse practitioner in a HIV clinical setting. Willing to travel to different sites in Lake County as needed (Umatilla, Leesburg, Clermont).
