Role Description The Direct Support Professional is a full-time, hybrid role based in Fort Wayne, IN, with some work-from-home flexibility depending on program needs and documentation requirements. In this role, you will provide person-centered support to adults with intellectual disabilities in home and community settings, assisting with daily living skills, personal care, and community integration. You will help individuals pursue their goals by supporting activities such as learning new skills, participating in recreational or vocational programs, and building social connections. Daily responsibilities include monitoring health and safety, implementing support plans, documenting services, and communicating regularly with supervisors, families, and other team members. You may also assist with transportation, participate in training sessions, and contribute to a positive, respectful environment that promotes dignity and independence. Qualifications Background or interest in Human Services and supporting adults with Intellectual Disabilities. Strong Interpersonal Skills and Communication abilities, including clear, respectful interaction with individuals, families, and colleagues. Willingness to participate in Training related to person-centered support, safety, and disability services. Ability to maintain accurate documentation, follow care plans, and uphold confidentiality and ethical standards. High school diploma or equivalent required; additional education in human services, social work, psychology, or related fields is a plus. Capacity to assist with basic physical tasks, follow safety protocols, and respond calmly in urgent or challenging situations.
