Job Description
Case Management - Acute Care

Schedule

Every other weekend required. Call is not required. May be required to float to meet department needs.

Assignment Details

Caseload of up to 25 patients.

Requirements

• 2 years of case management experience, with at least 1 year in acute hospital case management, current within the last year.
• Travel experience required.
• Valid state license required, compact license must be in hand at submission.
• Experience with Epic charting system required.
• Knowledge of discharge planning, care coordination, and utilization management.
• Experience with HMO, IPA, and DRG/benefits eligibility.
• Familiarity with advance directives/POLST and protective services process.
• References: 2 references covering 1 year within the last 3 years required.

Responsibilities

• Coordinate care and services for patients.
• Develop discharge plans to ensure smooth transitions of care.
• Manage resource utilization and advocate for patient needs.
• Assist in navigating benefits eligibility processes.
• Collaborate with interdisciplinary teams, including social services, to optimize patient outcomes.
• Conduct assessments and develop care plans according to patient needs.

Compliance / Notes

Facility provides housing resources and assistance if needed. COVID-19 vaccination is not required.
