PRIMARY JOB RESPONSIBILITIES

• Evaluates, interprets, plans, implements, and assesses patient care based on their age and medical condition.
• Efficiently manages the evaluation of new patients, coordinating care with a multidisciplinary team, and conducting follow-up visits as needed.
• Communicates information appropriately in line with organizational policies and discipline-specific guidelines.

JOB REQUIREMENTS

• Education: Must be a graduate from an accredited nursing program, holding either an Associate Degree, Diploma, or Bachelor of Science in Nursing.
• Licensure / Certifications: Must possess a current nursing license to practice in the state where care is provided (NC/SC).
• Experience: A minimum of two years of nursing experience is required, with a preference for home health experience.
• Technical / Motor Skills: Requires hand-eye coordination for administering injections, operating a computer, etc. Must be proficient in English communication and able to handle patient care equipment effectively.
• Interpersonal Skills: Ability to establish positive interactions with patients, their families, physicians, and staff to ensure effective patient care.
• Physical Requirements: Must be capable of transferring or maneuvering objects weighing at least 50 pounds as part of patient care. Frequent lifting, moving, and positioning of patients is necessary, along with the physical effort required for bathing and ambulating patients. Occasional overhead reaching, stair climbing, and fine motor skills are also required.
• Mental Abilities: Must be able to assess patient conditions, develop care plans, select appropriate interventions, evaluate responses to treatment, and educate patients about their conditions and recovery. Requires strong cognitive skills, including short- and long-term memory, prioritization, and the ability to learn new patient care techniques.
• Sensory Requirements: Ability to visually assess patients and utilize sight for implementing and evaluating care plans (e.g., changing dressings, starting IVs). Must also use hearing to auscultate lung and bowel sounds, respond to alarms, and communicate effectively with patients, families, and staff.
• Exposure to Hazards: Potential exposure to noise, bloodborne pathogens, body fluids, infectious diseases, and needle punctures. May encounter dangerous animals and traffic hazards during home visits, as well as situations that could pose a threat to personal safety. Must be prepared for temperature fluctuations and extreme weather conditions.
• Work Schedule: Typically Friday through Sunday, with the possibility of additional hours.
• Population Served: Adults, geriatrics, and pediatrics.
• Transportation: A valid driver’s license and access to a reliable vehicle are required.
