Position Summary We are seeking a compassionate and dedicated Registered Nurse (RN) to join our team and provide exceptional, resident-centered care in our skilled nursing community. As an RN Staff Nurse, you will assess residents' needs, administer medications and treatments, monitor changes in condition, and collaborate with physicians and the interdisciplinary care team to ensure the highest quality of care. Receive and provide thorough shift reports to ensure continuity of resident care. Assess, monitor, and document residents' physical, mental, and emotional status, reporting changes in condition promptly to the appropriate supervisor and physician. Administer medications, treatments, and therapeutic interventions safely and accurately while monitoring resident responses. Maintain accurate and timely clinical documentation, including medication administration, treatments, assessments, care plans, incident reports, and other required records. Coordinate and supervise the delivery of care by Certified Nursing Assistants (CNAs) and other nursing support staff, providing guidance and support throughout the shift. Collaborate with physicians, rehabilitation staff, and the interdisciplinary care team to develop, implement, and update individualized resident care plans. Admit new residents, complete nursing assessments, and assist with developing individualized plans of care. Respond promptly to resident call lights and address resident needs with professionalism, compassion, and respect. Accompany physicians during rounds, communicate resident updates, and accurately transcribe physician orders as needed. Monitor inventory and ensure the availability of medications, treatment supplies, equipment, and other nursing resources. Promote resident safety by following infection prevention practices, medication safety standards, emergency procedures, and facility policies. Assist with resident meals, nourishment programs, and other daily care activities as needed. Participate in the orientation and mentoring of new nursing staff and support a collaborative team environment. Serve as Charge Nurse or assume leadership responsibilities when assigned, ensuring efficient unit operations and quality resident care. Participate in quality improvement initiatives, mandatory education, and ongoing professional development. Maintain resident confidentiality and comply with all HIPAA, regulatory, and facility requirements. Perform other related duties as assigned to support the nursing department and provide exceptional resident care. Qualifications Graduate of an approved school of nursing required. Current New York State Registered Nurse (RN) license and registration required. Current BLS or CPR certification required. Experience in geriatric, medical-surgical, long-term care, rehabilitation, or skilled nursing preferred. Strong clinical assessment skills and the ability to develop, implement, and evaluate individualized resident care plans. Strong communication skills and ability to collaborate effectively with residents, families, physicians, and interdisciplinary team members. Compassion, patience, professionalism, and a genuine commitment to providing exceptional care to older adults. Ability to maintain accurate clinical documentation and follow nursing standards, facility policies, and regulatory requirements. Commitment to resident-centered care, resident safety, and promoting the highest quality of life. Ability to supervise, mentor, and support nursing assistants and other team members while fostering a collaborative work environment. Specific Requirements Must be able to read, write, speak, and understand the English language. Must possess the ability to make independent nursing decisions and exercise sound clinical judgment within the scope of practice. Must possess the ability to communicate and deal tactfully with residents, families, visitors, physicians, staff members, government agencies, and the public. Must demonstrate knowledge of nursing practices, procedures, professional standards, and regulations applicable to long-term care and skilled nursing. Must possess the ability to assess residents, develop and implement individualized nursing care plans, administer medications and treatments, and evaluate resident outcomes. Must complete and maintain all required documentation accurately and timely, including medication administration records, treatments, assessments, care plans, incident reports, and other required clinical documentation. Must maintain current licensure and continuing education requirements as required by New York State and professional standards. Must participate in facility education programs, mandatory in‑services, workshops, and professional development activities. Must maintain confidentiality and comply with all HIPAA, facility, and regulatory requirements. Must possess the ability to establish and maintain positive relationships with residents and their families. Must possess the ability to supervise nursing assistants and provide direction to support quality resident care. Must possess the ability to adapt to changing priorities, interruptions, emergency situations, and resident care needs. Must not pose a direct threat to the health or safety of other individuals in the workplace. Physical and Sensory Requirements Must be able to stand and walk for extended periods throughout the workday. Must be able to assist with lifting, positioning, transferring, and providing direct care to residents. Must be able to push, pull, move, and/or lift a minimum of 50 pounds. Must be able to frequently reach, bend, stoop, kneel, crouch, and perform other physical movements associated with resident care. Must possess adequate vision and hearing, or use appropriate adaptive devices, to perform the essential functions of the position. Must be able to cope with the mental and emotional demands of providing nursing care in a fast‑paced healthcare environment. Must meet the general health requirements established by facility policies. May be required to assist with emergency procedures, including resident evacuation, as needed. Working Conditions Works in a healthcare environment with regular interaction with residents, families, physicians, employees, and interdisciplinary team members. May work in resident rooms, nursing units, treatment areas, and other designated facility locations. Subject to frequent interruptions and changing priorities throughout the workday. Required to make clinical decisions within established nursing standards, facility policies, and regulatory guidelines. May be required to respond to medical emergencies, resident incidents, and other urgent situations. May be exposed to blood, body fluids, infectious diseases, air contaminants, hazardous chemicals, and other healthcare‑related hazards while utilizing appropriate safety precautions.
