Behavioral Health Community Guide Location: North and Central areas of New Jersey FLSA Status: Exempt Job Status: Full Time Work Model: Hybrid Work Schedule: Monday–Friday with occasional evenings and/or weekends Vehicle Required: Yes Amount of Travel Required: 25-50% within a 30‑mile community service area Reports To: Care Team Manager About the Position At Reema Health, we believe a personalized, community-based approach to care enables anyone to access the things they need. Our mission‑driven company seeks an experienced, self‑motivated Community Guide who has worked with individuals living with serious mental illness and substance use disorder. Responsibilities Engage and work with clients from diverse ethnic backgrounds, focusing on those with severe and persistent mental illness, substance use disorders, and complex medical conditions. Reach out to members and share about Reema Health to enroll them in our care model. Conduct cold outreach and manage a caseload of 60–100 members, providing coverage for other Community Guides during absences or increased workload. Assess member needs, support systems, and social determinants of health to create a patient‑centered care plan. Actively guide members through enrollment in behavioral and mental health services, including coordinating referrals and appointment scheduling. Collaborate with clinicians to help patients connect to care, join clinical visits, and follow up on social needs. Visit members in person as appropriate (home, community, hospital, or other settings). Utilize text, phone, email, and face‑to‑face interactions to match each member’s communication preference. Identify and connect members to community and health resources, including transportation to services. Document all interactions thoroughly and timely in the Reema App and EHR. Maintain confidentiality and adhere to HIPAA and confidentiality regulations. Represent Reema positively, serving diverse members with dignity and respect. Adapt to business needs as the role evolves. Education, Experience and Skills Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences or related field. 3+ years of direct experience working with the target population, or equivalent experience substituting for years of education. Experience with crisis planning, case management, boundaries, and personal safety in the community. Strong critical thinking, time management, strategic planning, and multitasking skills. Proficiency with technology, including Microsoft Office, G‑Suite, video conferencing, and EMS systems. Excellent verbal and written communication and interpersonal skills. Qualifications & Position Requirements 18 years of age or older. Valid driver’s license, vehicle insurance, reliable transportation; satisfactory record. Pass a pre‑employment drug screening and background check. Compliance with federal, state, and local regulations regarding patient confidentiality and HIPAA.
