OverviewProvides services including but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Works directly with physicians, other health professionals and patients to provide all aspects of patient care, including clinical and distributive tasks, for patients served within our health system. Helps patients and healthcare professionals achieve clinical outcomes and monitor progress toward common goals. Supervises other pharmacy staff in the absence of formal departmental leadership.
