Job Requirements

• Active Licensed Practical Nurse (LPN) license in good standing
• At least 1 year of nursing experience preferred in long-term care, clinic, or similar patient care settings
• Experience with patient documentation, electronic health records (EHR), and clinical workflows
• Strong communication, organization, and time management skills
• Ability to work collaboratively under RN supervision
• Comfort working in a patient-focused environment with varied clinical tasks
• Epic experience preferred

Responsibilities

• Provide direct patient care under the direction and supervision of the RN
• Review and document patient medical records, physician orders, and care activities accurately
• Assist with patient rooming in clinic and via telemedicine
• Call and confirm gastroenterology procedures as needed
• Review colon prep instructions with patients upon discharge
• Support electronic fax review and Epic workflow processes
• Review risk stratification forms and assist with patient portal messages
• Participate in unit teams, workgroups, and quality improvement initiatives
• Perform additional duties as assigned to support excellent patient care
