Position Highlights

Pay Rate: $ 59.86

Location: Tallahassee, FL

Position Overview

As a Clinical MTM Pharmacist, you will play a critical role in improving patient health outcomes through:

Conducting outbound patient calls (approximately 100 calls daily)

Performing Comprehensive Medication Reviews (CMRs)

Delivering in-depth consultations regarding statin medication use following AHA guidelines

Providing Medication Therapy Management (MTM) services

Reviewing patient medications and identifying opportunities for optimization

Accurately documenting prescription, diagnosis, and recommendation data into a web-based application

Collaborating with the MTM team to support quality and patient care initiatives

What We're Looking For

Active Florida Pharmacist License (Please include a screenshot of your Florida Pharmacist License at the top of your resume.)

Minimum 4 years of Clinical MTM Pharmacist experience

Experience conducting medication reviews

Strong understanding of pharmaceutical and medical terminology

Familiarity with Medicare Part D Star Measures, including:

• SUPD

• SPC

• CMR

Exceptional verbal and written communication skills

Fluently speak, read, and write English

Strong computer and technology skills

Preferred Qualifications

Management or leadership experience

Salesforce experience

Previous call-based MTM experience

Experience working in quality improvement initiatives

Responsibilities

• Conduct high-volume outbound patient outreach

• Complete comprehensive medication reviews

• Educate patients regarding medication adherence and statin therapy

• Document recommendations accurately and efficiently

• Support healthcare quality initiatives and Star Measure goals

• Participate in training and professional development opportunities
