Pharmacist - ClinicalJob Title: Pharmacist - ClinicalProfession: Pharmacy Specialty: OutpatientDuration: 13 weeksShift: Day 4x10-Hour (07:00 - 17:00); Mid 4x10-Hour (13:00 - 23:00)Hours per Shift: 10Experience: Three years in clinical hospital pharmacy or completion of a pharmacy residency.License: Current Registered Pharmacist license in CaliforniaCertifications: PGY-1 residency or equivalent experience preferred in relevant specialty areasMust-Have: Current California Registered Pharmacist license, Pharm. D. degree, clinical pharmacy experience in acute inpatient settings or outpatient settings.Description: This role involves reviewing orders for appropriateness and validating medication therapies. You will dispense medications and supplies in accordance with state and federal regulations. Consultation with healthcare providers to optimize therapy for patient safety is essential. You will answer inquiries from staff regarding medication administration. Guidance and training for pharmacy technicians, interns, and new pharmacists will be part of your responsibilities. Documentation and timely maintenance of patient charts are required for effective pharmacotherapy management. Participation in interprofessional rounds to enhance patient care is expected. Monitor patient medication therapies and intervene as necessary for optimal treatment. Engagement in patient education and medication reconciliation processes is necessary. You will evaluate and document the effectiveness of interventions to inform ongoing care. Development of guidelines and protocols to ensure safety and compliance with medication use is part of the role. You will serve in specialty areas as specified, providing expertise and training in pharmacotherapy. Collaboration with multidisciplinary teams to achieve desired patient outcomes is crucial. Preparation of compounded medications to meet quality standards may also be necessary. Documentation of clinical interventions and performance metrics will be part of your duties. Participation in formulary management and evaluation processes as assigned to support pharmacy operations is expected. Training and development for pharmacy residents and interns will be provided under your guidance. Assistance with patient referrals and coordination of care is an essential aspect of this role.
