- Implement and monitor rehabilitation plans for patients under supervision of a licensed PT
- Perform therapeutic exercises and modalities as prescribed; document patient responses and progress
- Assist with initial assessments and contribute to patient education on mobility, safety, and home exercise programs
- Support transfers, ambulation, gait training, and use of assistive devices while ensuring patient safety
- Maintain accurate, timely documentation in patient records; adhere to infection control and safety policies
- Collaborate with an interdisciplinary team (nursing, physicians, therapy, case management) to optimize patient outcomes

Requirements
- Active Physical Therapy Assistant license in Connecticut
- Ability to practice in a long-term care setting
- Availability for a 13-week travel assignment with guaranteed hours
- Strong communication and teamwork skills for interdisciplinary care
- Proficiency in basic documentation and charting practices
- Ability to work the listed shift (5x8 days)
