Job Description:

Schedule: Full Time Days 8:30am-5pm

Location: Coverage area: Field work covering Shawnee Mission area in their home setting.

  • Provides personal care to patients according to the assignment details and care plan.

  • Documents patient care and observations timely and appropriately.

  • Participates in end-of-life care for patients and families. Informs the registered nurse of any changes in the patient's condition.

  • Follows the schedule of care, including making several visits per day with travel.

  • Assists patients with activities of daily living, such as bathing, dressing, and grooming.

  • Monitors and records vital signs, including temperature, pulse, and respiration.

  • Supports patients with mobility and exercise as prescribed. Provides emotional support and companionship to patients. Maintains a clean and safe environment for patients.

  • Communicates effectively with patients, families, and healthcare team members.

  • Other duties as assigned.

Knowledge, Skills, and Abilities:

  • Multi task with changing scheduling [Required]

  • Must maintain at least 12 contact hours of education annually [Required]

  • Hospice Medicare Conditions of Participation [Preferred]

  • Interactions with stressful end of life issues [Required]

  • Computer Skills including excel, word [Required]

  • Good phone etiquette [Required]

  • Critical thinking skills [Required]

  • Knowledge of TEAMS meetings [Preferred]

  • Knowledge of EPIC patient software [Preferred]

  • Knowledge of telephone protocol and ability to operate a multi-line phone system [Preferred]

Education:

  • High School Grad or Equiv [Required]

Field of Study:

  • N/AS

Work Experience:

  • Experience in the health care field [Required]

  • Hospice experience [Preferred]

  • Basic Life Support - CPR Cert (BLS)[Required]

  • Driver's License (DL) [Required]

  • Certified Nurse Assistant (CNA) [Required]
