How you’ll make an impact in this role

• Drive patient outcomes by skillfully implementing and continuously monitoring personalized care plans.
• Serve as a vital communicator by accurately monitoring, documenting, and conveying changes in patient condition to the care team.
• Lead the coordination of care across multidisciplinary teams, acting as the central anchor to ensure a seamless and holistic patient experience.
• Execute medical orders from physicians and other providers promptly and precisely.
• Champion the transition home by proactively assessing discharge needs and coordinating necessary resources for a successful recovery after leaving our facility.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Oklahoma Board of Nursing required.

Education:

• Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.

What additional preferences we're seeking

• IBCLC certification - Preferred
