Growing together.The Field Based Community Health Worker is responsible for assessment, planning and implementing care strategies that are individualized by patients and directed toward the most appropriate, least restrictive level of care. They also identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services, and manage the care plan throughout the continuum of care as a single point of contact for the member. As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. The coordinator also addresses social determinants of health such as transportation, housing, and food access.Working Schedule: Schedule is Monday through Friday between the hours of 8 am to 5 pm. This position is a field-based position with a home-based office. You will work from home when not in the field.Location: Las Vegas and Surrounding CommunitiesIf you reside in Las Vegas and Surrounding Communities, you'll enjoy the flexibility to telecommute as you take on some tough challenges.Primary Responsibilities:Assess, plan, and implement care strategies that are individualized by patients and directed toward the most appropriate, least restrictive level of careCreate a positive experience and relationship with the membersUtilize both company and community-based resources to establish a safe and effective case management plan for membersCollaborate with patients, family, and healthcare providersIdentify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive servicesCommunicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all membersAdvocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare teamDocument all member assessments, care plan and referrals providedYou'll need to be flexible, adaptable and, above all, patient in all types of situationsEngage members either face to face or telephonicallyHelp members set person-centered SMART goals and develop a care plan to achieve those goals with regular follow up calls and ongoing documentation of progress towards goals metAdhere to detailed, specific documentation requirements in the member's health recordProactively engage the members to manage their own health and healthcare using Motivational Interviewing SkillsSupport the member to ensure pick-up of their RxPartner with care team (community, providers, internal staff) Knowledge and continued learning of community cultures and values May conduct Health Risk Assessments (HRA) if neededPartner with and support care team by completing delegated tasksProvide member education on community resources and benefitsUtilize strong skill sets of managing multiple tasks at a time, being self-motivated, driven toward quality results, managing time well, being very detailed oriented and organized, working well in a team and on your own, and the ability to manage multiple deadlinesKnowledge and continued learning of community cultures and valuesPerforms other duties as assignedYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear directions on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:High School Diploma/GED OR 5+ years of case management experience2+ years of experience working within the Community Health setting in a field-based healthcare role2+ years of experience or knowledge of the resources available, culture, and values in the communityIntermediate level of computer proficiency including Microsoft Outlook, Teams, and the ability to use multiple web applicationsAccess to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet serviceAbility to travel locally up to 50% of the timeMust reside within Las Vegas, Nevada or surrounding areasValid Nevada driver's license and current automobile insurance with access to reliable transportation that will enable you to travel to client and/or patient sites within a designated areaBilingual (Spanish and English)Preferred Qualifications:Community Health Worker (CHW) AccreditationLPN, CNA, CMAExperience working in Managed CareKnowledge of Medicaid/Medicare populationSolves routine problems on own. - Works with supervisor to solve more complex problemsPrioritizes and organizes own work to meet agreed upon deadlines.Experience working in team-based careSoft Skills:Strong communication and customer service skills both in person and via phoneAbility to work independently and maintain good judgment and accountabilityDemonstrated ability to work well with othersStrong organizational and time management skillsAbility to multi-task and prioritize tasks to meet all deadlinesAbility to work well under pressure in a fast-paced environmentExcellent verbal and written communication skills; The hourly pay for this role will range from $20 - $36 per hour based on full-time employment.
