Physical Therapist - Travel Contract

Active Indiana Physical Therapist license required to start the assignment. BLS certification from American Heart Association or American Red Cross required. Experience: open to all levels; preferred outpatient orthopedic experience. EMR experience: familiarity with Epic preferred. Client-specific requirements: ability to provide one‑on‑one outpatient orthopedic treatment, wear gray scrubs or facility‑branded apparel, complete contract personnel requirements prior to start. Assignment Details Facility Type: Hospital outpatient clinic (Outpatient Orthopedic Rehabilitation). Shift Name: Days. Shift Details: Monday–Friday, 08:00–16:30. Weekly Guaranteed Hours: 36. Call Requirements: none. Patient Type: general outpatient ortho, one‑on‑one treatments, may include walk‑in patients. EMR: Epic. Dress Code: gray scrubs or facility‑branded apparel required.
