Resource / Travel Social Services Director

Social Worker – Residential Care Signature HealthCARE provides integrated services including skilled nursing, rehabilitation, assisted living, and home‑based care across multiple states. Our resident‑centered care emphasizes social and emotional support. Job Details Full‑time position. Monday‑Friday travel with weekly overnight stays; all expenses covered. Responsibilities Complete the Social Services portion of the MDS, Resident Assessment Protocols, Resident Care Plan, and Progress Notes in a timely and accurate manner. Assist residents and families with social, emotional, and family issues. Participate in resident care planning by identifying social and emotional needs in accordance with the medical assessment. Maintain progress notes for each resident as required by company policy and state and federal regulations. Qualifications Bachelor's degree in social work. Preferred: 1 year of supervised social work experience in a health care setting. Long‑term care experience required. Willingness to travel up to 100% (including flying and/or driving).
