Job Status Casual Shift Varies Position Purpose Case Manager LMSW I to work in the Case Management Department at Grand Haven Hospital. What You Will Need Masters degree in social work required Prior employment experience in a related area preferred State of Michigan, social work license required What You Will Do Identify, coordinate communicate and participate in discharge planning with patients and a multidisciplinary team Recognize and monitor potential barriers to safe discharge planning and intervene as appropriate Navigate community resources as well as serve as a reference and liaison for community resources and service.
