Care Manager – Adult / Child Behavioral Health Location: Robeson County, NC – may reside anywhere near the county. Schedule: Hybrid/Remote with occasional travel required. Responsibilities Facilitate the care management process, fostering collaboration across integrated care teams. Conduct comprehensive assessments; share information and facilitate tailored care plans with the person supported and other team members. Monitor ongoing care plan implementation and facilitate updates at least every 12 months or when circumstances significantly change. Implement workflows, policies, and documentation standards that promote whole‑person care. Maintain a positive, professional image in the workplace and community. Follow policies to ensure accountable service delivery. Continuously improve and define innovative care management interventions, keeping the team informed of best‑practice advances. Collaborate cross‑functionally with other Integrated Care Team members, community organizations, and RHA departments. Apply a person‑centered philosophy, focusing on strengths, needs, and preferences to promote health, wellness, and safety. Promote person‑centered culture within Care Management Teams, supporting education around self‑management and integrated healthcare planning. Use data strategically to support effective, integrated care management services and ensure consistent data sharing and storage. Adhere to all organizational health and safety policies and procedures. Document services in a timely manner for accurate reimbursement. Maintain frequent, regular communication with persons supported, family members, team members, and colleagues; facilitate open communication and feedback. Establish and maintain strong relationships with external providers and community organizations, including Clinically Integrated Networks (CIN). Support an ethical culture in alignment with the RHA Code of Conduct, emphasizing confidentiality and no retaliation. Investigate allegations of abuse, neglect, exploitation, and other violations of ethics and compliance policies, ensuring protection of individuals served and adherence to reporting requirements. Participate in quality improvement, quality assurance, safety, and human rights activities as assigned. Perform other related duties as assigned. Qualifications Education & Experience – Care Managers must meet at least one of the following criteria to be a Qualified Professional per 10A NCAC 27.0104A: A MH/SU license (including associate level) or certification by the NC Substance Abuse Board. An RN with ≥4 years of full‑time experience working with the MH/SU/IDD population. A master’s degree in a human services field with ≥1 year of full‑time experience working with the MH/SU/IDD population. A bachelor’s degree in a human services field with ≥2 years of full‑time experience working with the MH/SU/IDD population. A bachelor’s degree in a non‑human services field with ≥4 years of full‑time experience working with the MH/SU/IDD population. Licenses & Certifications – Valid driver’s license required; clinical license preferred. Experience in care coordination, care management, or case management. Essential Physical & Sensory Requirements Lifting: regular lift of ≤10lbs; must lift minimum 15lbs and pull minimum 10lbs. Movement: squat, kneel, crawl, crouch, climb, and stoop comfortably. Hand use: regular use of hands to finger, handle, or feel objects, tools, or controls. Standing & walking: required to stand and walk regularly. CPR proficiency: demonstrated CPR competence from the floor level. Vision: adequate close, distance, and peripheral vision.
