Position Summary Position responsible for identifying, triaging, and co-managing patients with medical and behavioral health problems within the primary care setting by providing the Primary Care Behavioral Health (PCBH) model. The PCBH model is designed to provide population-based care services within the setting of primary care. The goals include guiding patients through the GATHER framework: Generalist, Accessible, Team player, Highly productive, Educator, Routine. Your contribution to the team includes: Collaborate with primary care providers and other team members to recognize and treat mental health conditions, substance use/misuse, and psychosocial problems. Provide population-based behavioral health care, including establishing routine patient care and triaging patients during warm handoffs. Refer cases to mental health specialists when appropriate and bridge care until patient connects with traditional services. Conduct brief behavioral health visits (30 minutes typical; warm handoff 15–30 minutes) and schedule follow-ups based on clinical need. Communicate with team members about patient care, providing consultation and recommended interventions. Teach patients evidence-based interventions to improve self-care, preventive medicine, quality of life, and functionality. Lead group classes and visits to promote skill building and health improvement. Monitor the program, identify service issues, recommend quality improvements, and link to community resources. Participate in meetings, quality improvement activities, and site committees as required. Respond to complaints and resolve disputes positively. Support the growth of Behavioral Health services, including supervising trainees as allowed by state licensing. Maintain good attendance, professionalism, and uphold HealthPoint’s mission and core values. Complete additional duties as assigned by supervisor. Qualifications Doctoral degree in Psychology. Licensed Psychologist in Washington State. Experience in a health care/medical setting (required). Maintain current psychology licensure with Washington. Basic computer skills: Word, Excel, Outlook.
