This role provides safe and competent quality care based on nursing theory and research. Delivers patient care by incorporating the principles of Relationship Centered Care. Applies the nursing process based on theory, research, and evidence-based practice. Assesses and evaluates patient care needs. Utilizes critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with interdisciplinary healthcare teams in developing and implementing care plans. Prioritizes all aspects of patient care, including education and discharge planning. Educates patients and caregivers about their plan of care, health promotion, and disease prevention. Communicates professionally with patients, families, and all members of the healthcare team. Manages assignments effectively within the care delivery model. The role encompasses patient experience responsibilities. Model the principles of Relationship Centered Care. Considers patient values, preferences, and cultural diversity in care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to improve understanding of patients' perceptions of care. Handles service recovery for patients and families with concerns. Demonstrates empathy and active listening in interactions. Conducts purposeful rounding to address patient needs consistently. Maintains quality and safety in patient-centered care. Identifies and escalates barriers to improvement in patient safety and quality. Demonstrates clinical skills crucial to maintaining quality and safety standards. Implements process improvement strategies based on evidence.
