Registered Nurse - Care Manager

Job Summary and Responsibilities As our RN Care Manager, you will strategically coordinate the care of hospitalized patients, optimizing resource utilization and meticulously planning for safe and timely discharges that meet individual patient needs, all while upholding the highest standards of professional nursing. Every day you will collaborate closely with a multidisciplinary team, including social workers, providers, utilization review specialists, and ancillary departments. Through this collaboration, and the expert application of care guidelines, you will actively ensure patient progression along established clinical pathways. A critical aspect of your role involves creating a seamless and safe transition for patients into appropriate post‑acute settings. To be successful in this role you will possess exceptional clinical assessment skills, possess a deep understanding of care coordination and discharge planning, and exhibit strong communication, negotiation, and critical thinking abilities to navigate complex patient needs and healthcare systems efficiently. Job Requirements Required Current Washington state licensure as an RN. BLS from AHA upon hire. Bachelor of Science of Nursing. Strong patient assessment skills, ability to work collaboratively in a team setting, current working knowledge of electronic medical records. Preferred/Desired Experience managing complex patients or patients with chronic disease in an ambulatory setting.
