Physical Therapist (2556) - Citywide - H00079 Full-time, Permanent Civil Service. Under general supervision, the Physical Therapist provides physical therapy evaluations and treatments to a diverse client population. Duties Perform physical therapy evaluations, including assessment of functional status and physical condition. Develop treatment plans including goal setting. Administer appropriate interventions and procedures, including education and training of caregivers. Communicate professionally with other care team members. Document per regulatory and Practice Act guidelines. Meet all administrative requirements, including attendance of staff meetings and record‑keeping. Oversee care provided by physical therapy assistants in all settings and CHHAs in the home environment. Provide clinical case management and supervision over the home health care team when appropriate. Participate in patient care meetings and performance improvement projects as assigned. Explain treatment objectives and methods to patients and their families, including instruction on use of prosthetic and orthotic devices and other equipment. Instruct students‑in‑training and auxiliary personnel in elementary procedures and techniques, and coordinate rehabilitation with physicians, nurses, teachers, occupational therapists, speech‑language pathologists, and others. Qualifications Licensed by the Physical Therapy Board of California. Valid Basic Life Support for Professional Rescuer (BLS) certificate issued by the American Heart Association (must be obtained within two weeks of hire).
