Home Health Aide (HHA)Facility: Seagate Rehabilitation and Nursing Center Location: Brooklyn, NY Schedule: PRN – Any Shift Pay: $20/hourAbout The PositionSeagate Rehabilitation and Nursing Center is seeking a compassionate and reliable Home Health Aide (HHA) to escort residents to and from clinic and dialysis appointments while ensuring their safety, comfort, and well-being.ResponsibilitiesEscort residents safely to and from clinic and dialysis appointments.Arrive 1 hour prior to scheduled clinic appointments.Assist residents with preparing for appointments and provide out-on-pass snacks.Monitor residents for safety and help prevent falls or altercations.Report accidents, incidents, or concerns immediately to the Clinical Coordinator or ADNS.Ensure required paperwork and medical documentation accompany residents.Verify DNR/DNI status and required documentation.Advocate for residents and communicate clinic concerns to the facility.Maintain professional conduct and follow facility policies and abuse-prevention regulations.Coordinate with the Clinic Coordinator regarding assignments and appointments.May be required to attend multiple clinic visits in one day.QualificationsCurrent NYS HHA certification required.CNA certification preferred.High school diploma or equivalent preferred.Minimum 1 year of EMR experience; Sigma experience preferred.Prior nursing home experience preferred.Professional appearance and strong communication skills required.Join Our Team!If you are dependable, compassionate, and committed to providing excellent resident care, we encourage you to apply today!
