Forest City Nursing and Rehab is seeking a PRN Physical Therapist (PT) to provide high-quality rehabilitation services at our skilled nursing facility in Forest City, PA. Your Responsibilities:

• Deliver evidence-based physical therapy services within your scope of practice.

• Participate in evaluations and assist in developing personalized treatment plans.

• Implement targeted interventions that support mobility, strength, and functional independence.

• Document care accurately and communicate resident progress with the interdisciplinary team.

What You'll Bring:

• Current Pennsylvania licensure as a Physical Therapist.

• BLS certification.
