Provide skilled nursing interventions to address physical, emotional, and psychosocial needs; adjust plans as needed to optimize outcomes. Serve as the primary point of contact for patients, families, and the care team to ensure clear communication and continuity of care. Collaborate with physicians, nurses, social workers, and other staff to coordinate transitions of care, referrals, and discharge planning. Document assessments, plans, interventions, and outcomes accurately in the medical record; ensure compliance with regulatory guidelines. Monitor patient progress, re-evaluate plan effectiveness, and participate in quality improvement initiatives as part of the care management process.
