Position Description
Responsible for coordinating transportation and transportation of residents to and from physician appointments in a timely
manner and under the supervision of licensed nursing personnel.
Principal Responsibilities
• Organizes transportation of residents to complete required appointments based on priority of resident needs.
• Transports resident to and from inpatient and outpatient programs and to and from appropriate facilities and
residences.
• Develops driving schedule based on resident’s scheduled medical appoints and program destination depending on
resident appointment needs; Provides transportation of current or new residents from hospital after discharge.
• Provides report, information, and assessment of resident’s behavior or change of condition during transportation
upon return to center; Reports changes of abnormal findings in resident status such as consciousness, pain, bleeding
and vital signs that occur during the lift or transporting; Observe residents carefully and reports changes in condition
to clinical supervisor.
• Transports medical records for signatures to medical offices.
• Maintains a high level of safety following safety policies and procedures while in Center and transporting residents.
• Always maintains van in a neat and clean condition; Maintains van in good working condition; Ensures that routine
maintenance on vehicle is performed, logs mileage, and that vehicle is clean.
• Assist clinical staff with safe lifting, transferring, and/or transporting resident as specified by the physician orders
and/or clinical supervisor; Assist in the care of residents requiring moderate to total assistance in moving.
• Establishes positive communications with outside facilities, hospitals, physicians' offices, and /or specialty clinics to
ensure proper services for residents are maintained and scheduled appropriately.
• Promptly reports any scheduling changes to the DON; nursing staff and ancillary service providers, responsible
parties and takes follow up action, as necessary.
• Provide adequate hydration to residents at regular intervals each day according to specific resident needs.
• Protect the personal property of the residents of the facility.
• Other duties, responsibilities and activities may change or assigned at any time with or without notice.
General Responsibilities
• Attends and participates in scheduled training, educational classes, and meetings to maintain current certification as
applicable; Attends and participates in in-service training as mandated by regulatory agencies and company policy.
• Adheres to work schedules in completing and performing assigned tasks; completes all assigned tasks in a timely
manner as instructed by the direct supervisor.
• Meets acceptable center attendance requirements; notifies facility before late arrival to or absence from work
according to established center policy.
• Cooperates with other facility personnel to ensure that services can be adequately maintained to meet the needs of
residents; creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm
environment.
• Adheres to established employee policies.
• Adheres to established dress code.
Customer Service Responsibilities
• Models customer service principals throughout the center and promotes appreciation of our customer’s needs.
• Treats residents, residents’ family members, visitors and fellow employees with courtesy, respect, and dignity.
• Presents professional image through appearance, behavior, and speech.
• Explains procedures to residents before administering services.
Residents Rights
• Understands, upholds, and promotes the rights of the residents.
• Ensures residents can exercise rights without interference, coercion, discrimination, or reprisal from the facility.
• Ensures protected health information is kept confidential.
• Ensures resident concerns/complaints are responded to with tact and urgency.
• Reports allegations of resident abuse, neglect and/or misappropriation of resident property.
• Coordinates effective communication with Residents and Companions by assuring all arrangements for providing
interpreters and/or other auxiliary aids and services needed by Residents and Responsible Party are made, including
scheduling interpreters through outside agencies and scheduling staff interpreters, if any; and ensuring appropriate
maintenance, repair, replacement and distribution of auxiliary aids.
Safety and Sanitation
• Follows established safety policies and procedures.
• Identifies and corrects safety hazards; reports safety concerns as required.
• Observes safety needs of patients as indicated in care plan.
• Wears and/or uses safety equipment and supplies when indicated and properly trained to use.
• Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations.
• Reports work-related injuries as soon as reasonably possible to supervisor and human resources department; is
responsible for information regarding injury, correspondence and forms from workers' compensation or subsidiary
plan carrier or records received from medical provider. Follows and actively participates in the return to work
program as established by the Company.
• Complies with workplace protections such as pre-employment and continuous employment screening, testing and
infection control including TB testing, flu outbreak prevention and SARS-CoV-2, virus that causes COVID-19,
prevention.
Staff Development
• Attends and participates in scheduled in-service training, educational classes, and meetings to maintain current
certification as applicable and as managed by regulatory agencies and company policies.
• Ensures that small group meetings are held per company policy.
• Communicates new Policy & Procedures and regulations to staff to ensure compliance.
Qualifications
• Certified in the state of practice, required.
• Successful completion of State Approved Nurse Aide Training and Competency Evaluation Program; Name must
appear on State Registry as Nurse Aide in good standing.
• Current BLS/CPR certification (All SNFs; ALFs as required by state).
Travel
• Significant. Will escort residents to outside appointments.
Principal Contacts
• Works closely with residents, nursing staff and clinical supervisors.
