Your primary responsibility will be to gather essential information from clients, assess their needs, and guide them through the initial stages of our support process. This position requires excellent communication skills, attention to detail, and the ability to handle sensitive information with care and confidentiality.Key ResponsibilitiesAnswer incoming phone calls and respond to email inquiries in a timely and professional mannerConduct initial assessments to determine client needs and eligibility for various programs and servicesAccurately input client information into our database management systemExplain available services, programs, and resources to clientsSchedule appointments and follow-up consultations for clients with appropriate staff membersMaintain detailed and organized client recordsCollaborate with other departments to ensure smooth client transitions and continuity of careAssist in the development and implementation of intake procedures and policiesParticipate in team meetings and contribute to improving the intake processMaintain confidentiality and adhere to HIPAA regulations and other relevant privacy lawsProvide basic emotional support and crisis intervention when necessaryGenerate reports on intake activities and client demographics as requiredStay updated on available community resources and referral optionsAssist with administrative tasks related to the intake processQualificationsRequired:Home care intake experienceStrong phone etiquette and verbal communication abilitiesProficiency in using computer systems and database management softwareAbility to multitask and prioritize in a fast-paced environmentEmpathetic and patient demeanor when interacting with clientsBasic understanding of confidentiality practices and privacy regulations
