Job Summary:

Seeking a dedicated Physical Therapist to provide comprehensive outpatient rehabilitation services. This role involves developing individualized treatment plans to improve patient mobility and function.

Responsibilities:

• 
  Evaluate patients physical conditions through detailed assessments and establish personalized therapy goals.
  
  
• 
  Implement evidence-based treatment plans to enhance mobility, strength, and overall patient wellbeing.
  
  
• 
  Monitor patient progress and modify therapy techniques as needed to ensure optimal outcomes.
  
  
• 
  Educate patients and their families on therapeutic exercises and injury prevention strategies.
  
  
• 
  Document patient sessions, progress notes, and treatment outcomes accurately and promptly.
  
  
• 
  Collaborate with healthcare teams to coordinate patient care plans and facilitate smooth therapy progression.
  
  

Qualifications:

• 
  Valid state or federal issued identification required.
  
  
• 
  Current license or credential as a licensed Physical Therapist.
  
  
• 
  Proven experience in outpatient physical therapy settings preferred.
  
  
• 
  Strong knowledge of therapeutic techniques, mobility assessment, and patient care standards.
  
  
• 
  Excellent communication and interpersonal skills for effective patient interactions.

  Ability to work independently and as part of a multidisciplinary team.
