Requirements:

Board Eligible or Board Certified in Otolaryngology REQUIRED
Current or Willing to obtain Colorado Medical License REQUIRED
Interest in practicing General ENT full-time REQUIRED
Availability/start date REQUIRED
Clean malpractice/license history - HIGHLY PREFERRED
License history and disciplinary history REQUIRED
Candidate's subspecialty interests (Laryngology, Skull Base Surgery, etc.) HIGHLY PREFERRED
Clean malpractice/license history - HIGHLY PREFERRED
Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at the presentation
DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - REQUIRED no exemptions
Visa Support: Does provider require J1 or H1B support? (note in "Additional Submission Details" field)- REQUIRED
