Program Coordinator

Job Title: Program Coordinator Department: Medical Clinic Work Division/Location: McCloud/Mt. Shasta/Dunsmuir FLSA Status: None exempt Classification: Full-time/Part-time Reports To: Clinical Operations Manager/COO $25.00/hr - $31.00/hr Summary/Objective: The Program Coordinator plays a vital role in supporting patients and families as they navigate healthcare, social services, and community resources. This position focuses on building trusted relationships, helping individuals overcome barriers to care, and connecting them with the services and supports they need to improve their health and well-being. The Program Coordinator works closely with patients, care teams, community partners, and local programs to ensure coordinated, compassionate support. In addition, this role helps identify gaps or challenges in access to care and shares insights with leadership to support continuous improvement and better outcomes for the communities we serve. Essential Functions: Ability to maintain confidentiality Support public health efforts to educate the community on infection diseases Deliver information about health and wellness in ways that the community can easily understand Establish positive and supportive relationships with patients and those who work within the community/county programs to foster positive experiences and outcomes Motivate patients to be active and engaged in their health Ability to objectively report data and identify targeted areas of program activity Ability to document activities as required within the provided systems Other duties as assigned ECM Coordinator Responsibilities: The ECM Coordinator will be instrumental in structuring the Enhanced Care Management program, in conjunction with the Clinic Operations Managers/COO for Shasta Cascade Health Centers. This position will be working in collaboration with a Community Health Worker to contact and structure check-ins with clients. This role will be front facing with patients and families to determine if patient(s) qualify for the ECM program and then support in educating and accessing appropriate programs for their needs. All clients will be provided a structured plan of care with goals that align with the ECM program. This position requires a majority of patient-facing time in the community, as well as time spent collaborating with the various programs in the community that may serve as resources for patients, such as community resources centers, food banks, mental health services, public health services and more. Supervisory Responsibilities: This position has supervisory responsibilities. Work Environment: This job operates in multiple environments: professional medical clinic environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This position will also operate in the community which will be a variety of indoor and outdoor settings; ie: community/senior centers, community organizations, resource fairs, etc. Travel: Travel is expected for regular job duties. Travel may occasionally be required for training purposes. If personal vehicle is used for specified job-related errands or travel to/from training events, mileage will be reimbursed as specified in the Employee Handbook. Qualifications Education, Experience, Certifications, Licenses and Registrations: High School Diploma or GED required Two years working in social work, community outreach, or customer service Must successfully complete an approved Community Health Worker program within 12 months of hire Basic Life Support (BLS) certification, or ability to become certified.
