Summary: Supports the mission of the hospital by aiming to reduce hospital readmissions through patient discharge optimization and follow-up. Performs clinical duties and engage in patient care to assist with medication safety, increases patient medication and disease state understanding, and encourages provider follow-up and continuity of care following hospital discharge.

Minimum Qualifications:

• Other information: Preferred Experience is PGY-1 residency; PGY-2 residency.
• Education: Essential: * Bachelors Degree Nonessential: * Doctoral Degree
• Education specialization: Essential: * Pharmacy Nonessential: * Pharmacy
• Education equivalent experience: Essential: * Required - Bachelors - Pharmacy; Preferred - Doctorate - Pharmacy
• Credentials: Essential: * Pharmacist Credential equivalent experience: Essential: * Required - Pharmacist - State of Florida; Preferred - ASHP (Pharmacists)

Shift Hours: 8AM-4:30PM
