Field-Based Housing & Care Lead Coordinator
Pacific Health Group
Pacific Health Group is seeking a field-based Housing Specialist to support CalAIM Community Supports and housing programs across Siskiyou County. You will travel locally, perform assessments, and coordinate housing resources to help members achieve stable housing. You’ll work independently in the community, build partnerships with landlords, shelters, and providers, and document in real-time per Medi-Cal guidelines. #J-18808-Ljbffr Pacific Health Group
