Opportunity Step into a pivotal position that combines compassion with expertise, as you facilitate Medicaid eligibility and coordinate essential services, enhancing the quality of life for individuals requiring long‑term care support. Responsibilities Determine initial and ongoing Medicaid eligibility for long‑term services and supports (LTSS). Develop and implement client service plans. Coordinate the provision of services for community‑based care and nursing homes. Regularly review and update Medicaid eligibility and monitor service plans. Minimum Qualifications A bachelor’s degree in a Behavioral Science, Social Science, or a closely related field; or A bachelor’s degree in any field and one year of human services related experience; or An associate degree in a Behavioral Science, Social Science or closely related field and two years of human services related experience; or Three years of human services related experience. Essential Attributes Excellent communication skills to discuss and assess client needs. Strong people/customer service skills. Experience obtaining information through interviews. Experience preparing and maintaining written documentation of actions taken or needed. Background Checks and Requirements Selected finalists will undergo a criminal history and background check. Adverse criminal history may lead to disqualification. A valid driver’s license and an acceptable driving record are required. Working Conditions The office is open from 8a.m. to 5p.m., Monday through Friday. Duties include traveling to individuals’ homes, hospitals, nursing homes, and other provider settings. Employees may encounter physical exertion, uneven terrain, environmental hazards, uncontrolled situations, animals, and contagious diseases such as COVID‑19, MRSA, and staph infections.
