About the Position

Job Title: Home Health RN Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of Certified Home Care experience and 1 year of Case Management experience License: Valid RN license Certifications: CPR Certification preferred Must-Have: - 1 year of Certified Home Care experience - 1 year of Case Management experience - Ability to travel to cases as assigned Description: All applicants must have 1 year of Certified Home Care experience. 1 year of Case Management experience is required. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Weekend rotations are expected once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. Required to perform admission processes and case management responsibilities in accordance with agency policies and standards of practice. Utilizing thorough and timely electronic documentation and interdisciplinary communications is essential.
