Nurse Navigator - Heart Failure Clinic, PRN

Position Overview The Nurse Navigator supports patients with heart failure by coordinating care, providing education, and ensuring seamless transitions across the continuum of care. Responsibilities include assessing patient needs, developing evidence-based care plans in collaboration with interdisciplinary teams, and monitoring progress to improve outcomes and reduce hospital readmissions. This role promotes a patient-centered approach, emphasizing safety, quality, and operational efficiency within the Heart Failure Clinic. Key duties involve facilitating care transitions, supporting disease management, documenting accurately in electronic systems, and engaging patients and providers to enhance access and education. The Nurse Navigator acts as a clinical resource, tracking outcomes, participating in quality improvement initiatives, and fostering a collaborative, growth-oriented environment.
