Care Coordinator

Day in the Life of a Care Coordinator Meet face-to-face with patients to facilitate recovery services and document all activities in the patient record. Utilize collateral contacts and local referral agencies to provide services not offered by the program. Conduct monthly Care Coordination meetings with patients and complete corresponding documentation to ensure engagement with needed services. Work collaboratively with the buprenorphine-waivered practitioner prescribing buprenorphine or naltrexone for patients with moderate to severe opioid use disorder. Perform other duties as assigned by the Operations Director of OBOT Services. Responsibilities Assist patients in creating and following recovery plans. Maintain accurate and timely documentation in patient records. Coordinate referrals to ancillary services and agencies. Attend Care Coordination meetings and support patient engagement. Collaborate with prescribing practitioners to monitor treatment progress. Education and Experience Requirements Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling. At least one year of experience providing services to individuals with a diagnosis of substance use disorders, or a minimum of one year of clinical experience working with individuals with co-occurring substance use disorders and mental illness. Licensure as a Registered Nurse with at least one year of direct experience providing services to individuals diagnosed with a substance use disorder, or a minimum of one year of clinical experience with co-occurring diagnoses. Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC‑Assistant under supervision of a CSAC or another supervisor as defined in 18 VAC 115‑50‑10 Schedule.
