Works in both inpatient and outpatient settings. Responsibilities Patient Navigation and Care Coordination. Guide patients and families through the cancer care continuum. Coordinate diagnostic testing, consultations, procedures, and care transitions. Perform patient assessments via phone or in person and implement nursing interventions. Qualifications Registered Nurse with experience in oncology or related critical/chronically ill patient care. Strong clinical assessment and decision‑making skills.
